Isles of Sea and Sky Secret Hint Mechanics (SPOILER)
Take the first island as an example.
This will only happen if no secrets have been revealed in the room.
Snakes can reveal secrets like hidden stairs, hidden passages, and sheet music (I see a lot of people asking where the sheet music is) and their eyes will close once all the secrets in the room have been found.
(Same as the example below, hidden staircase found and no red eyes)
As for the notes, this block will have an almost transparent note emblem before you select it.
So keep your eyes peeled if you see the snakes eyes turning red. Notes are especially hard to find when they are on ice blocks.

Exception (spoiler alert)
However, there is at least (or only?) one exception, a hidden passage that is not indicated by snakes.
(But it’s a black gem, so it doesn’t have any achievement or termination requirements? So feel free to look it up yourself.)
It’s on an island, you can only get to it with a giant bird ride. (Lower Left of World)
The exact passage is given below. I also highlight the “hidden passage/staircase” marker with a red rectangle.
If you see the mark in other places, try pushing everywhere!
